Medical Advocacy Services

A recent Johns Hopkins study claims more than 250,000 people in the U.S. die every year from medical errors. Other reports claim the numbers to be as high as 440,000.

Vital Care Partners promotes dignity and independence for our clients while providing peace of mind for family members. We understand that there are challenges when our family members age and we guide our families to solutions with knowledge and compassion. We are excellent problem solvers, providing practical solutions and guidance to our clients and families every step of the way. Vital Care Partners advocacy evaluates every medical, environmental, psychological and social aspect of a person’s life and examines how these elements ultimately interact with each other. We integrate all these elements to ensure a comprehensive plan that provides a more balanced, enriched and fulfilled life within their capabilities. The Vital Care Partners approach is effective across all ages, genders, races, cultures and lifestyles, and has consistently proven to change the course of a person’s overall quality of life. We are successful in working with those whose issues are immediate and critical as well as those whose challenges are chronic. We also provide ongoing quality assurance through our oversight program. Oversight of care, either in the home or in a facility, is done through regular, unannounced visits, with surprise spot checks, by our highly trained professional staff of RNs, to ensure all care orders are being followed exactly as specified.

  • Medical Oversight- attending doctor’s appointments, monitoring medication, arranging for medical services.

  • Management and care for chronic illnesses.

  • Patient Review for Nursing Home Admittance.
